Social Work Services


Social Workers with the Huron Shores Family Health Team (HSFHT) work as members of a multi-disciplinary team to encourage and support a holistic approach to enhancing the physical and mental well-being of patients.

​Patients rostered with the HSFHT can access individual counselling with our social workers for the following reasons:
​
  • Short-term therapy to support psychosocial and mental health needs of patients by enhancing problem-solving skills, coping and developmental capacities. Sessions provide psychoeducation and focus on future focused goal setting. (Adult service)
  • System navigation to link patients with resources, services and opportunities for all patients
  • Consultation with other HSFHT members to coordinate care and maximize outcomes
  • Complete cognitive assessment (MOCA & MMSE)
  • Support referrals to psychiatry
