Diabetes is a complex disease that can be impacted by factors in and out of our control. Our team is prepared to help provide you with the tools, resources, and knowledge needed to care for it appropriately. "Diabetes" refers to when the sugar in our blood gets too high because insulin is either not being made or is made in less amounts or your body becomes resistant to its effects. If left untreated or not managed, diabetes can result in a variety of complications including heart, kidney and eye disease and nerve damage.
The Diabetes Care Team works with you to help develop a plan to control and manage your diabetes. Controlling diabetes can help prevent or delay its damaging long-term complications. Our team works with adults 18 years and older who live with pre-diabetes, type 1 diabetes, and type 2 diabetes. How frequently you follow up with each team member may depend on your progress, needs and blood sugar control. |
Diabetes Nurse EducatorAssists you in understanding how your medications work, how to prevent complications, and how to use diabetes devices, such as glucose meters and insulin pens. They coach you through what to do when your blood sugars get too low and how to prevent them from getting too high.

Chiropodist/Foot CareOur Chiropodist and Foot Care Nurse assist you in understanding how diabetes affects your feet, detect and treat foot problems. Having regular foot health monitoring can significantly reduce your risk of serious foot complications

Registered DietitianRegistered Dietitian's (RDs) in Ontario are trained food and nutrition experts. Our RD is dedicated to assisting you in understanding how your diet affects your diabetes and to consider the options available, then assist you to make a plan to help you achieve your goals.
